Changes in questionnaires

I do have a couple of requests for the site based on feedback from the participants:

• In the breast cancer survey, please make the following edits in red in the attached document. There are some participants that don’t have breast cancer and are hesitant to complete the survey due to numerous questions referring to breast cancer diagnosis. There are also a series of questions missing in the survey.

• When the new participant is signed up and first time logged into the system, we currently show him "Informed Consent" questionnaire before redirecting to the dashboard.
  We need to modify that behavior and after the informed consent show 'Breast Cancer Study Survey' and after that 'Authorization for Release of Medical Images'.
  Potential solution - add all these questionnaires into one wizard component.
  Please, discuss alternative options with Elena.
