Improve offline reporting capabilities

Improve the excel template file for easier accessibility, formatting and readability for offline reporting excel sheet.

• change name of the export file to Report_PdRefNoPortionAfterLastSlash.xlsx e.g. QPR1_PD2018124.xls
• I am attaching a mock up, where:
  add a readme sheet, that is the same for all reports highlight only the fields that must be filled in by the users (in orange or other agreeable colour)
  delete or hide unnecessary columns - they are stricken-through in the spreadsheet change the tile of some of the columns (the text changes are highlighted in red font - in the final version it should be black font - i used red to highlight for the developer only).
